Licensing, Registration & Certifications per state and facility requirements.
Evidence of successful completion of an ANP / CRNA program of education.
Have a minimum of two years of experience of routinely practicing as a CRNA.
Have current certification in basic cardiac life support (BLS / ACLS) for health care workers.
Certifications must be obtained through an American Heart Association approved training program and must be hands on courses.
No felonies.
No flagged or under investigation licenses.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
